I'm guessing if she had been wildly unstable, it wouldn't have gotten to that meeting, but I don't know how academia works -- if you're on the tenure track, do you automatically stay there?

Pretty much. Except many places now have a (usually 3rd/4th year) review to make sure you are on track to get tenure, basically to avoid the stress of "you've worked here 7 years, but now you're fired" that is getting denied tenure.
