Also, in discussing this position with a few people who are on the periphery of it, I have come to understand that the fall is the busiest month for this office. When should I mention that I have a 2 week vacation in October scheduled and booked? Now? Second interview? During the offer negotiations?

I just called my general physician's office to schedule an appointment for a blood draw. Apparently, I am no longer a patient. Since I've healthy and haven't been into the office in four years, I no longer count.
But I'm welcome to reregister as a new patient although it'll be a few weeks before I can get an appointment. Feh.
Not before you have an offer. It's part of the "when can you start" conversation, not part of the "why should we hire you and not someone else" convo, unless it's the kind of job where "we need a warm body by Monday"

I called my gyn's office. They're actually humans there and since this was an issue documented during the surgery, they have all the background info. But it's really annoying to discover that you think you've had a regular physician this whole time and you kind of haven't.

yeah, you would think that they would let you know.
Once I opened a checking account, and then a week or two later I ran my balance down to $0. Then I found out my account had been closed, because they had a policy to automatically close a new account if the balance was $0. WTF?
At least they reopened my account without any hassle.

The baby slept for five consecutive hours last night, I've had a shower, and my stitches must be dissolving because I can suddenly stand up straight today. IOW, I feel almost human, again.
Have I mentioned here that I was happy she was born on the 18th, because I kept thinking in hospital that Anne Bolyn lost her head on May 19th? I'd have to look it up to be sure.
