Stereotypical Girl Code would say yes, I guess, but I think in real life it depends on the relationships involved. Have sex with someone I had sex with? Eh. Start dating my ex I'm not quite over yet? That's a different story.
Yeah. There are several people that I've slept with who have also slept with friends of mine (hi, we are lesbians, it's a small world) but it's more of an issue to sleep with someone I was super into, and maybe am not over. Also, I think it would be weird to date someone who I had known as a friend's partner for a long time. Like, not "oh, Mary and Lucy have been dating a few months" but like "I went to Mary and Lucy's wedding a few years ago, because I went to college with Mary, but they just broke up and now I want to date Lucy"
