If you have expenses whose cost and dates you can predict, do the math and sock away as much as you can afford. I tended to put away my deductibles plus a little more, but you can spend it on pharmacy purchases as well. I never got organised enough to do that, not least of all because it took years for my meds to stabilise.

It might depend on the plan, but the one I'm on locks you in for a lump sum for the year. It doesn't really matter when in the year you spend it.

Absolutely. The flip side is you spend it or you lose it, so a case like yours where you have predictable expenses makes for the budgeting good.
